THE HEART CENTRE

The Heart Centre at Sharif Medical City Hospital sets the standard for premier heart care. The cardiac surgeon and the cardiologist uses the best existing technology for cardiac surgery. The department is equipped with modern operating rooms, Cardiac Intensive Care Units with one to one nursing care for each patient, angiography lab and CCU. Continuous 24-hour care for our patients is provided by a team of highly knowledgeable and experienced professionals, including cardiac surgeons, cardiologists, intensive care specialists, anaesthetists and specialist nurses. Our cardiac Surgeon, Dr. Saeed Afridi has extensive experience with the vast variety of cases and handled over 3000 cases since his career started in cardiac surgery. He is experienced in his field, skilled in addressing everything from the most routine to rare, complex procedures.
Currently we provide FREE surgical treatment for heart patients who are not affording or from remote areas of country. We are doing 20 free heart surgeries each month with excellent success rate. We provide surgical treatment of a very wide range of complex cardiac problems which include:

INTRODUCTION

A corneal transplant is the process of removing all, or part of, a cloudy or abnormally shaped cornea and replacing it with a healthy donor’s cornea. Corneal transplants are mostly performed to improve vision, however if the cornea has perforated (or threatens to perforate) due to disease or trauma, then a transplant may be required to maintain the structural integrity of the eye.
The cornea is the clear window at the front of the eye, its main function is to focus and transmit light. Injury, disease, or hereditary conditions can cause clouding, distortion, or scarring of the cornea, which may all interfere with vision.
The service of Corneal Transplant at Sharif Medical City Hospital is supervised by one of the leading Eye surgeons of Pakistan, Prof. Dr. Samina Jahangir.

WHEN IS A CORNEAL TRANSPLANT NEEDED?
A corneal transplant is required for a variety of eye conditions, the most common being;

Corneal clouding: There are many conditions that may cause the cornea to become swollen and cloudy, such as Fuchs’ endothelial dystrophy and pseudophakic bullous keratopathy. In severe cases the cornea may develop small blisters on the surface which can be very painful. Corneal infection: Infections of the cornea may be bacterial, fungal, or viral. Severe corneal infections may cause scarring, thinning, or perforation of the cornea. Herpes simplex is the most common viral infection that may lead to scarring.
Corneal dystrophy: Some hereditary conditions called dystrophies cause the cornea to become opaque or irregular and may require corneal transplantation.
Corneal injury: Injuries to the cornea, such as penetrating eye injuries, may cause corneal scarring that affects the vision and requires corneal transplantation.
Your ophthalmologist will assess your need for a corneal transplant by looking carefully into your condition.

TYPES OF CORNEAL TRANSPLANTS At the Sharif Medical City Hospital, we perform both the standard full-thickness corneal transplant called the penetrating keratoplasty, as well as more advanced and newer techniques called lamellar keratoplasty. Your ophthalmologist will assess which procedure is most suitable for you.
Penetrating Keratoplasty (PK): This is a full-thickness corneal transplant used to treat diseases that affect the full thickness of the cornea. Endothelial Keratoplasty (DSAEK, DMEK): This is a partial-thickness corneal transplant used to treat diseases where only the endothelium (the back layer of the cornea) is affected, such as Fuchs Endothelial Dystrophy.
Anterior Lamellar Keratoplasty (ALK, DALK): This is a partial thickness corneal transplant used to treat diseases which affect the stroma (the front layers of the cornea), such as Keratoconus or certain dystrophies.

THE PROCEDURE Corneal transplant surgery usually takes about an hour and is performed under a general or local anaesthetic. Fine instruments are used to remove the affected cornea and it is replaced with a clear donor cornea. Fine nylon stitches are used to suture the graft into place. In the case of endothelial keratoplasty, an air bubble is used to attach the graft into place. Most patients go home on the day of the surgery, though some may require an overnight stay.

RECOVERY
A pad or plastic shield will be placed over the eye after the operation and is left in place until the eye is examined the following day. Eye drops will be prescribed for several months to years in order to reduce the chance of infection and rejection. Some sutures will be removed as early as four weeks, but suture removal can take up to one year for most people. There is usually mild to moderate discomfort after the surgery, it is very unusual for there to be severe pain. Your ophthalmologist will explain your expected recovery time and vision quality in detail.

RISK & COMPLICATIONS When using modern surgical techniques, the risks are small, however, as with any operative procedure, occasional complications can occur. For more information on the possible risks and complications make an appointment with one of our ophthalmologists.

DONOR CORNEAS Every cornea is thoroughly checked for transmissible diseases, including HIV and Hepatitis B and C.Your ophthalmologist will advise you of the availability and waiting times for a donor cornea. You can find additional information by contacting our transplant secretariat.

We feel privileged to announce that Sharif Medical City will be performing ONE free Cataract surgery (Sutureless surgery through latest phacoemulsification technique) every day. We will also be performing ONE completely free of cost CORNEAL TRANSPLANT every week. This service is especially initiated to help the poor and deserving segment of our society.

INTRODUCTION

Think how it feels like to be born with stigma of cleft lip/palate in a developing country like Pakistan. Cleft patients are handicapped in multiple aspects of life because of dilemma of refusal and denial by their own communities. SCCLAP is a centre of its own kind, headed by Principal Prof Col(R) Iffat Batool Syed (HOD Orthodontics),who is a renowned orthodontist .She has been dealing these patients with ultimate care and empathy since 30 years. She treats cleft patients absolutely free of cost. Cleft disorders seldom present without dental occlusal anomalies which necessitates orthodontic treatment.

Over 300 patients have been treated by Prof Col Iffat ; in which infants orthopedics has taken the lead. The usage of obturators and other prosthetics devices facilitate feeding as well as nasoalveolar moulding of the cleft patients. We also address psychological turmoil of parents and guides the mother that how parenting can be done in these special cases. Orthodontist also monitor a child’s speech development and reports any delays or errors noticed without further delay.

Skeletal discrepancies of these patients are corrected either by dentofacial orthopaedics in growing years or complex cases by maxillofacial surgery.

Treatment of cleft patients is multidisciplinary which needs a very systematic and efficient approach in all related disciplines . This schematic treatment planning and timely intervention makes this cleft centre the biggest and state of art.

TREATMENT MILESTONES FOR CLEFT PATIENTS Birth: Initial Assessment, Pre-surgical assessment, NAM (starting as early as 2 weeks)
3 Month: Primary Lip repair provided patients have 10 weeks of age, 10 pounds of weight and 10 g of haemoglobin.
9-18 months: Palate is repaired. 2 years: Speech assessment by orthodontist and thus referred to speech therapist. 3-5 years: Lip revision surgery is done if required. This is assessed by orthodontist.
8-9 years: Initial interventional orthodontics (growth modification) and preparation of alveolar bone grafting by orthodontist. This facilitates the eruption of canine which is cornerstone in an arch.
10 year: Alveolar bone grafting.
12-14 year: Comprehensive orthodontics is done as there is hardly any cleft patient presenting no malocclusion.
16 year: Nasal revision surgery.
17-20 year: Orthognathic surgery when the growth is over. TYPES OF CLEFTS: Unilateral: Cleft is only on one side of lip or palate or both.
Bilateral: Cleft is present on both sides of lip or palate or both. Usually nasal deformity is much enhanced in this.
Cleft lip: Cleft is only of lip.Lip taping is enough.
Cleft palate: Cleft is only of palate. Alveolus remodelling is very important aspect of treatment.
Cleft lip and palate: Both lip and palate are involved in cleft. NAM , Lip taping with obturator and columellar lengthening is required.

TECHNIQUES FOR ACTIVATIONS OF OBTURATORS: Following gold standard techniques are followed for activation of obturators.
Figueroa’s NAM reconstruction: Lip taping and acrylic relief to the palatal plate to approximate alveolar gap.
Grayson’s NAM reconstruction: Acrylic plate to mould the alveolar segments and extension to mould alar cartilage of nasal component.
WHAT WE ACHIEVE? Lip approximation: Aids surgeon for an aesthetic lip surgery. Non surgical columellar lengthening: Minimal surgical invasion required for nose uplifting
Alignment of cleft segments: better approximation achieved for bone grafting.

THE PROCEDURE:
INFANTS ORTHOPAEDICS – NAM:
Lip taping: Customised lip taping is done at initial visit.
Obturator: Putty Impression is taken and model and finally acrylic plate is formed. Soft relining is done to prevent trauma to baby.
Activation: Activation is done weekly.

ADOLESCENCE – FUNCTIONAL JAW ORTHOPAEDICS As maxillary growth is restricted in cleft patients , therefore face mask is given to redirect growth of maxilla in a forward direction. Palatal expanders are given to expand the constricted maxillary arch which is usual feature of cleft.
ADULTS:
Pre surgical orthodontics followed by orthognathic surgery.
Tertiary bone grafting if required.

RISK & COMPLICATIONS When using modern surgical techniques, the risks are small, however, as with any operative procedure, occasional complications can occur during impression procedure but presence of experienced orthodontist minimises the risk.

Chronic Kidney Disease is a major global health problem. In Pakistan, one in eight persons is estimated to have chronic kidney disease. Chronic kidney disease if not detected and managed early can progress to kidney failure which requires treatment with dialysis or kidney transplant.

Sharif Medical City Hospital offers both hemodialysis (cleaning of blood via machine) and peritoneal dialysis (removal of toxins by instilling fluid inside belly). In addition, other services related to dialysis including temporary and permanent hemodialysis catheter placement, creation of AV fistulas and grafts and peritoneal dialysis catheter insertions are also provided at the hospital.

Currently, Sharif Medical City Hospital has 22 functional dialysis machines which include one machine to remove extra fluid in patients and one dedicated machine in intensive care unit which can be modified to perform slow and long dialysis in critically ill and unstable patients. There are two separate dialysis units for hepatitis C positive and negative patients, where patients, staff and dialysis machines are completely isolated thus reducing the risk of transmission of hepatitis C infection among patients. A dedicated machine in an isolated room is reserved for hepatitis B positive patient. Sharif Medical City hospital also has a satellite dialysis centre in Faisal Town, Lahore which is equipped with four hemodialysis machines.

In 2016, more than 11,000 dialysis sessions were performed in the dialysis units of the hospital, which makes it one of the largest dialysis service provider in Lahore in private sector. Nearly half of these sessions were done free of cost while rest were provided at an affordable price. Currently Sharif Medical Trust provides free of cost hemodialysis facility to 50 patients, with an intention to expand this service to 100 patients in near future.

Obstetrics & Gynaecology

Sharif Medical City Hospital is a 459 bedded tertiary care centre situated at Jati Umra Raiwind road Lahore, Pakistan. It is a referral centre. Obstetrics and gynaecology department of Sharif Medical and Dental College is a 50 bedded unit which provide services 24/7.
It caters for all the major and advanced conventional surgeries and also has the facility of endoscopic surgeries both diagnostic and operative laparoscopy. Our supporting Paediatric Department is equipped with neonatal ventilators. Babies as small as 28 weeks are successfully delivered and survived. Epidural analgesia and antenatal/intranatal fetal monitoring are available round the clock. Outreach clinics are run in Raiwind city. Hospital is providing 2/week free of cost major gynaecological operative services for deserving patient from Chairman’s Patient Benevolence Program.

OBSTETRIC SERVICES
Obstetrical services provided by department includes:

GYNAECOLOGICAL SERVICES
- Management of simple gynaecological cases such as vaginitis, -cervicitis, abnormal vaginal bleeding, family planning and contraception, vaginal and uterine prolapse, infertility cases.
- Screening for malignant disease.
- Provision of Fertility Services & Uro- gynaecological Services ·
- Emergency gynaecological services in cases of ruptured ectopic pregnancy or ovarian cyst accidents.
- All gynaecological operations including minor such as dilatation and curettage/ Manual vacuum aspiration either for diagnosis or for miscarriages, cervical cerclage & cautery, bartholin abscess and cyst.
